Once a family is confirmed as eligible for the relevant funding, the parent/carer is required to complete a parent/carer agreement form for each funding period and for a specific age group:
- 9 to 23 months parent/carer agreement form (PDF)
- 2 year old funding parent/carer agreement form (PDF)
- 3 and 4 year old funding parent/carer agreement form (PDF)
- Optional: 3 and 4 year old funding parent/carer agreement form for universal/EYPP only (PDF)
You will use the information detailed on these forms to complete your Synergy provider portal online submission, according to the dates set out in the headcount and payments timetable.
Parents/carers should be issued with a privacy notice to advise what you will do with their child’s information.
