Better Care Fund overview
The Better Care Fund Plan is joint plan between the Council and local health services to improve the ways adult social care and health services work together, starting with services for older people and people with long term conditions. Our plan set out a shared vision for health and social care in Central Bedfordshire, rooted in a locality-based delivery model. It describes the agreed strategic approach based on four programmes for delivering integrated care and improved outcomes for older people in Central Bedfordshire.
Our vision is for the people of Central Bedfordshire to have access to good quality, safe, local health and social care across its towns and rural areas. This must be centred on the integration of health and social care through a whole systems and seamless approach to improving physical and mental health, so that people can experience “care without organisational boundaries” and ‘better care, locally’.
Our ambition is for local people to be able to say:
My care is planned with people who work together to understand me and my carer(s), put me in control, co-ordinate and deliver services to achieve my best outcomes.
